Enrollment Method

1. Register in person at the school
2. Enrollment Procedures on the Park’s Website.
3. Registration by Mail (Send a copy of required documents and a check for the registration fee by mail)
   Please prepare
   – Application form (can be obtained from the school or downloaded from the website)
   Birth certificate (original or copy)
   – Acupuncture card (original or copy)
   – 2 photos
   – 1 self-addressed envelope
